I understand that tooth gems are non-invasive and semi-permanent. If I decide I no longer want my tooth gem(s), I will visit a licensed dentist for proper removal. Due to Texas dental regulations, Houston Tooth Gems does not remove tooth gems.*
I understand that tooth gems placed on the curved edge of a tooth or on lower teeth experience more pressure during chewing and may not last as long as gems placed on the flat surface of an upper tooth.*
If my tooth gem falls off within 3 months of placement, I will notify Houston Tooth Gems promptly. Houston Tooth Gems will reapply the gem or replace it one time at no cost. I understand that a dentist may need to remove any remaining dental composite before a new gem can be placed in the same location.*
I understand that the 3-month replacement guarantee does not apply to client-provided gems or gems placed on veneers or crowns. After 3 months, the reapplication fee is $50.*
I will inform Houston Tooth Gems if I would like a gem placed on veneers, crowns, bridges, or any surface other than natural enamel. I understand that special preparation is required and the bond may not be as strong as with natural tooth enamel.*
If receiving teeth whitening, I agree to avoid smoking and consuming foods or drinks that may stain my teeth for at least 24 hours after treatment, as teeth are more susceptible to staining during this period.*
I understand that Houston Tooth Gems and Battle Royale Tattoo are not responsible for damage to natural or artificial teeth during or after tooth gem application or teeth whitening services.*
I confirm that I do not have untreated cavities, gum disease, or active oral infections.*
(If under 18yrs old) I confirm that I am the parent or legal guardian of the minor receiving this service and I give permission for Houston Tooth Gems to perform the requested service. I understand that I must be present for the appointment and accept all terms on behalf of the minor.

Entiendo que las joyas dentales (tooth gems) no requieren perforar ni modificar el diente y son semipermanentes. Si decido que ya no quiero mi joya dental, acudiré a un dentista con licencia para que la retire correctamente. Debido a las regulaciones dentales de Texas, Houston Tooth Gems no retira joyas dentales.*
Entiendo que las joyas colocadas cerca del borde curvo de un diente o en los dientes inferiores reciben más presión al masticar y pueden durar menos tiempo que las joyas colocadas en la superficie plana de un diente superior.*
Si mi joya dental se desprende dentro de los primeros 3 meses después de su colocación, avisaré a Houston Tooth Gems lo antes posible. Houston Tooth Gems volverá a colocar la joya o la reemplazará una vez sin costo. Entiendo que puede ser necesario que un dentista retire cualquier resto de resina dental antes de colocar una nueva joya en el mismo lugar.*

Entiendo que estas superficies requieren una preparación especial y que la adhesión puede no ser tan fuerte como sobre el esmalte dental natural.*
Si recibo un blanqueamiento dental, acepto evitar fumar y consumir alimentos o bebidas que puedan manchar mis dientes durante al menos 24 horas después del tratamiento, ya que durante este período los dientes son más susceptibles a las manchas.*
Entiendo que Houston Tooth Gems y Battle Royale Tattoo no son responsables por daños a dientes naturales o artificiales durante o después de la colocación de joyas dentales o de los servicios de blanqueamiento dental.*
Confirmo que no tengo caries sin tratar, enfermedad de las encías ni infecciones activas en la boca.*
(Si el cliente es menor de 18 años) Confirmo que soy el padre, la madre o el tutor legal del menor que recibirá este servicio y doy permiso a Houston Tooth Gems para realizar el servicio solicitado. Entiendo que debo estar presente durante la cita y que acepto todos estos términos en nombre del menor.
